Make sure you fill in all the required areas with the asterisk.  ->  *

For the date on the bottom part of the form, write the day when you won the prize.

(Ex. If you won a prize on January 1st, then write January 1st even if you send us the e-mail on January 10th.)

We cannot send you the prizes if any part of the consent form is not filled in,

so make sure you look thoroughly before sending us the form.

Please make sure you write the country when you write down your address.

When you send us the consent form via e-mail,

make sure you write your nickname and the country you're in on the title of the e-mail.
